How to add NPS surveys to Gravity Forms
Gravity Forms can collect an NPS score with an 11-option Radio Buttons field, or a Survey field when the Survey Add-On is active. For automatic promoter, passive and detractor grouping with tracking, use an NPS-focused add-on.

Define the NPS questions
Use the standard NPS question: How likely are you to recommend our business, product or service to a friend or colleague? Give respondents a scale from 0 to 10. Scores of 0–6 are detractors, 7–8 are passives and 9–10 are promoters. NPS is calculated as the percentage of promoters minus the percentage of detractors.
Add a second question such as What is the main reason for your score? Keep it optional if you want the shortest possible form.
Build the manual version
For a basic version, edit your form and add a Radio Buttons field. Enter eleven choices numbered 0 through 10, then make the field required. Add a Paragraph field for the written follow-up question.
This route works without the Survey Add-On, but Gravity Forms will store the answers rather than give you a dedicated NPS report. You will need to group the scores and calculate the result yourself from the entries.
Use the Survey field instead
If you have the Gravity Forms Survey Add-On, add a Survey field from the Advanced Fields section. Set its Survey Field Type to Likert, enter the NPS question, replace the default choices with 0 through 10, and make the question required. Add a separate Paragraph Text field for the reason behind the score.
The Survey Add-On provides survey-specific fields and a Survey Results view, but its standard survey reporting is not the same as a dedicated NPS dashboard. Review what it reports before promising automatic NPS totals to your team.
Show relevant follow-up questions
Use conditional logic to show the written follow-up only when it is useful. For example, display a feedback field for scores below 7, or show a referral question for scores of 9 or 10. With Survey fields, conditional logic for Likert responses requires Gravity Forms 2.8.1 or later and Survey Add-On 4.0.0 or later.
Check the condition after saving the form. A common mistake is comparing the visible label with a different stored value, which prevents the follow-up field from appearing.
Test entries and calculate results
Submit test entries for 0, 6, 7, 8, 9 and 10. Confirm that the score is saved, the follow-up field appears under the intended conditions, and notifications contain the answers you need.
For the manual route, export or review the entries, count promoters and detractors, divide each count by the total number of valid responses, and subtract the detractor percentage from the promoter percentage. Do not include blank or incomplete responses in the calculation.
Take the fast route
For automatic NPS handling, install and activate the Gravity Forms NPS Survey Add-On, then add its NPS field to your form and configure the 0–10 question. Add a Paragraph field if you want written feedback, and use the add-on’s response tracking to review promoter, passive and detractor results in the WordPress dashboard.
This is the better option when you need repeat surveys, trend tracking or results that other people can read without maintaining a spreadsheet. The manual Radio Buttons version is still fine for a small form with occasional responses.
